Personalized Care That Evolves with Your Loved One’s Needs

Hospice care is not one-size-fits-all. At Rapha Care Hospice, we provide four distinct levels of care, each tailored to match your loved one’s medical condition, comfort, and support requirements. These levels are designed to evolve as needs change—ensuring continuity, dignity, and compassion every step of the way.

1. Routine Home Care

This is the most common level of hospice care. It allows patients to receive regular nursing, medication, and supportive services in the comfort of their home—whether that’s a private residence, assisted living facility, or nursing home. Our care team visits on a scheduled basis to manage symptoms, provide personal care, and offer emotional and spiritual support.

2. General Inpatient Care

When symptoms become too severe to manage at home, we provide short-term inpatient care in a hospital or skilled nursing facility. This level is recommended for patients experiencing acute pain, respiratory distress, or other complex conditions requiring 24-hour nursing care and close medical supervision.

3. Continuous Home Care

For patients needing intense support at home due to a crisis or rapid decline, we offer continuous nursing care—up to 24 hours a day. Our interdisciplinary team manages pain, breathing issues, or other urgent symptoms while helping the patient remain at home, avoiding hospitalization when possible.

4. Inpatient Respite Care

Hospice recognizes the emotional and physical toll caregiving can take. Respite care provides up to five consecutive days of temporary relief in a contracted inpatient setting. This ensures the patient continues to receive quality care while the caregiver rests and recharges.

How to Choose the Right Level of Care

Selecting the appropriate level of hospice care should be a collaborative decision between your family, our hospice team, and the patient’s physician. Most patients will experience more than one level of care as their journey progresses. All levels include access to nursing services, medical supplies, medication management, spiritual support, and social work services.
