We deliver and maintain all necessary equipment—such as hospital beds, oxygen, and mobility aids—so families can focus on care, not logistics.
Palliative Care
For patients facing serious illness, our palliative care team offers relief from symptoms and stress. Care is coordinated with the patient’s other providers to ensure comfort and clarity in every decision.
Spiritual & Emotional Support
We care for the whole person. Our chaplains and counselors provide spiritual care and emotional support that honors each family’s beliefs, traditions, and personal values.
Grief & Bereavement Support
We support families long after their loved one has passed. Our bereavement program offers counseling, check-ins, and memorial resources for up to 13 months after loss.
Volunteer Services
Trained volunteers offer meaningful companionship, reading, conversation, or caregiver relief. Their presence helps bring warmth, peace, and human connection to the bedside.
Social Work Services
Our licensed social workers assist with emotional support, advance care planning, and community resources. They help patients and families navigate tough decisions with clarity and compassion.
Eligibility
Hospice care is available to individuals diagnosed with a life-limiting illness and a life expectancy of six months or less, as determined by a physician. Patients must choose comfort-focused care rather than curative treatment.

How to Start Care
Getting started with hospice doesn’t have to be overwhelming. At Rapha Care Hospice, we walk with you every step of the way to ensure a smooth, compassionate transition into care.
Call Us
Contact our 24/7 hospice intake team to speak with a care coordinator. We’ll answer your questions and explain the process.
Referral & Assessment
A referral can be made by a physician, hospital, family member, or even the patient. Our nurse will visit to assess eligibility and begin building a care plan.
Plan of Care
Once admitted, our team will collaborate with your doctor and family to create a personalized plan focused on comfort and quality of life.
